Responsibilities:

• Lead the inter-disciplinary team in Resident Care Conferences to ensure the social, psychological, and emotional needs of each resident and family member are met.
• Oversee appropriate assessment prior to Initial Care Conference according to Minimum Data Set guidelines.
• Maintain ongoing MDS assessments and care plans in related areas.
• Complete documentation in every resident's record through care plan reviews and pertinent daily recordings.
• Provide direct social work services to residents and family members, including counseling, group work, problem-solving, information and referral, financial assistance, legal assistance, and advocacy to facilitate adjustment to the facility.
• Carry out social service duties in accordance with current federal, state, and local standards, guidelines, regulations, and established policies and procedures.
• Ensure that the medically related emotional and social needs of the resident are met and maintained on an individual basis.

Specific Requirements:

• Must be able to read, write, speak, and understand the English language.
• Possess the ability to make independent decisions when circumstances warrant such action.
• Demonstrate tactful interpersonal skills when interacting with personnel, residents, family members, visitors, government agencies/personnel, and the public.
• Exhibit patience, tact, a cheerful disposition, and enthusiasm, while adjusting interactions based on the maturity level of residents.
• Be open to seeking new methods and principles to incorporate into existing Social Service practices.
• Communicate relevant information regarding a resident's condition/incident to the Interdisciplinary Team as appropriate.

Experience:

• Minimum of one (1) year of social work experience in a hospital, skilled facility, or other health-related field preferred.

Education:

• Must possess, as a minimum, a BSW from an approved school of Social Work.
